Fatality Prevention

Every day at Tenke Fungurume Mining we complete tasks that are considered to be high risk tasks. What are these tasks?

  • Site Wide High Risk Tasks: Operating a light vehicle, Mass Transit, Pedestrian/Equipment Interaction, Cranes and rigging, Energy Isolation, Exposure to Open Holes, Contact with Electricity, Working with or handling HDPE, Mobile Equipment operation, Blasting, Fatigue Management, Stockpile Operation, Work at Heights, Confined Space, Excavation and Trenching.
  • Fatality Prevention: Do we do these tasks safely and properly each and every time?. How do we know if we are doing these tasks safely and correctly?. We measure certain outcomes and certain precursors to determine how well we are doing.
  • Outcomes: Some of the outcomes, otherwise known as lagging indicators, that we measure include: Injury rate, Property Damages, Near Misses, Potential Fatal Events.
  • Precursors to a fatality: What is a precursor? A precursor is one of the leading indicators that we measure. A precursor is a warning sign that substandard conditions or behaviors exist that could lead to a fatality.
  • Lessons Learned: Potential Fatal Events and Fatal events are almost always caused by employees not following the proper procedures or by not stopping a task and asking for help when they aren’t sure or when the job changes. What can we do?: Always follow proper procedures; If you aren’t sure of the proper procedures, STOP and ask for help; If the job changes, STOP and make a new plan.  Ask for help to recognize new hazards.
  • Before you begin a task you should answer the following questions: Satisfied that you know all the risks?; All the right PPE and tools are on hand and in good condition?; Forms and permits needed are filled out and signed?; Each person has enough time to complete the job safely?; Talked to each other about the job?; Your work area is safe?; If you answer “No” to any question you must not start the task until you have discussed the issues with your supervisor and you can answer “Yes” to all questions.
